Responsibilities

• Engages and motivates members to participate in case management programs and assists with family-centered care.
• Collects information for tailored assessments and refers onward when criteria for referral are met.
• Provides navigation support to access social determinants of health and coordinate transitions of care.
• Outreach to PCP/POCs, specialists, and home care to facilitate referrals and build self-management skills.
• Answers calls, schedules appointments, and maintains program records and case files.
