Responsibilities

• May lead duties for other team members including training and managing day-to-day operations.
• Refers families to clinical, social, and financial supports using an established algorithm.
• Works with staff and providers to create a care plan and communicates status of referrals.
• Engages with diverse populations to reduce cultural and socio-economic barriers.
