Qualifications

• Associate's Degree in Health Care or Social Services required.
• 1 year of experience in a health care setting with emphasis on customer service, including scheduling, registration or insurance focus.
• Two years of relevant experience in a health care setting with emphasis on customer service in lieu of degree, in addition to the required experience.
• Experience with medical forms, letters, and prior authorizations preferred.
• Experience with data entry and electronic medical records preferred.
• Experience working with individuals from diverse ethnic, cultural, and socioeconomic backgrounds preferred.

Responsibilities

• Partner with care team members to coordinate patient care.
• Navigate patients through the health care system and support patients receiving specialized care.
• Work with patients and families to ensure appropriate scheduling for clinical needs.
• Connect patients and families to clinical, social, and community resources.
• Provide emotional support and education within the specialty care discipline.
• Collaborate across the health care team to coordinate and medically manage specialty care needs.
