Confluent Health LLC is seeking a Care Coordinator to be the first voice and face for patients, ensuring a warm, organized, and professional experience from initial contact through visits.

You will manage front desk duties, scheduling, insurance verifications, and patient communications while supporting a fast-paced clinical team. Training is provided, with opportunities for growth within the clinics.

Qualifications

• High School Diploma or GED.
• At least one year of experience in a customer-facing or patient-facing role.
• Ability to manage multiple tasks while staying organized and detail-oriented.
• Comfortable communicating on the phone with professionalism.
• Basic computer proficiency: scheduling software, emails, spreadsheets.
• No prior medical experience required; training provided in EMR navigation and scheduling workflows.

Responsibilities

• Create a welcoming, professional first impression for patients and visitors.
• Handle front desk operations: check-in/check-out, scheduling, insurance verification/authorization, phone calls.
• Review and follow up on referrals; update patient records and explain next steps to patients.
• Keep patient flow moving by confirming appointments, managing waitlists, and handling cancellations.
• Collect copays and other payments accurately; basic payment tracking.
• Uphold HIPAA standards in all interactions and documentation.
