Peninsula Post Acute is seeking a reliable Case Manager Assistant to join our team in a fast-paced healthcare setting. You will support the Case Management department with administrative tasks, care coordination, and timely services for residents and families.

Qualifications

• Experience in healthcare, skilled nursing, rehabilitation, or social services preferred.
• Strong organizational and multitasking abilities to manage multiple residents and tasks.
• Excellent communication and customer service skills with residents and families.
• Dependable with strong follow-through and attention to detail.
• Ability to prioritize responsibilities in a fast-paced healthcare setting.
• Proficient with computers and standard office applications and HIPAA awareness.

Responsabilités

• Assist Case Managers with daily administrative duties and coordination tasks.
• Communicate with residents, families, hospitals, healthcare providers, and community agencies.
• Support discharge planning and coordination of post-discharge services.
• Maintain accurate documentation and resident records.
• Assist with referrals, appointments, transportation arrangements, and follow-up services.
• Coordinate information between departments to ensure timely completion of tasks.
• Monitor outstanding items and follow through to completion.
• Maintain confidentiality and comply with HIPAA and facility policies.
• Provide professional, compassionate support to residents and families.
• Perform additional duties as assigned to support the Case Management department.
