Job summary

PACS Group seeks a Patient Care Coordinator to collaborate with residents, families, clinicians, and insurers to develop a cost-conscious, patient-centered plan of care. You’ll coordinate admissions/discharges, track progress, and ensure timely documentation.

Excellent communication, strong organizational skills, and understanding of Medicare/Medicaid concepts are essential. This is a full-time, on-site role supporting post-acute care teams across facilities.

Qualifications

• Bachelor’s degree in a related field is preferred (non-licensed candidates accepted).
• Healthcare, case management, or social services experience is strongly preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• Familiarity with PCC or other EMR systems preferred.
• Strong communication skills with ability to present information to residents, families, and staff.

Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment protocols.
• Attend morning meetings to facilitate team communication.
• Answer questions about care to ensure residents feel safe and secure.
• Monitor and adjust residents’ statuses based on changing needs.
• Organize and prioritize daily work by assessing residents’ needs.
• Complete documentation as required.
• Coordinate care with residents, providers, facility financial services, and third‑party payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Relate positively with residents, families, staff, and colleagues.
• Ensure accurate charting and timely paperwork.
• Work independently and exercise sound judgment with physicians and payers.
