You will act as a liaison among staff, coordinate bed management, and facilitate admissions, discharges, and transfers. You will also serve as a unit-based resource for hospitalist teams, manage records, and greet patients and families, handling routine inquiries and coordinating

Responsibilities

• Bed management: tracks, prioritizes and assigns beds; coordinates admissions, discharges and transfers; communicates with BMD and BM; updates discharge/admission logs.
• Maintains and updates patient records, organizes chart for Medical Records and transfers; prepares discharge records and signs Medicare forms as needed.
• Operates as unit-based resource for Hospitalist teams; assists rounds, initiates calls and coordinates coverage and post-discharge follow-up.
• Greets new patients/families, assists staff, coordinates staffing for next shift and informs departments of urgent tests and discharge needs.
• Performs unit records/log tasks: maintains charts in order, prepares records for transfer/discharge, and manages unit audit tools.
