Job summary

Concord Hospital is seeking a Patient Care Coordinator to support patient registration, referral coordination, and front-desk duties. The role ensures timely, efficient, and compassionate customer service for patients and families under supervision of Practice Administration.

The ideal candidate has high school education with proven customer service experience and strong knowledge of medical office operations including scheduling, EMR, and payer guidelines.

Qualifications

• High School diploma or GED; annual registration competency exam required.
• Strong customer service experience in medical settings preferred.
• Knowledge of medical office operations: scheduling, registration, EMR, referrals, payer guidelines.

Responsibilities

• Obtain all prior auths / precerts for the Cardiovascular Institute. This includes testing and procedures.
• Obtaining and managing VA insurance referrals.
• Obtain when necessary, PCP insurance referrals.
• Responsible for the timely management of precert workqueue.
• When necessary, obtain and verify insurance and demographic information for referrals and precerts.
• Processes and routes all incoming calls or requests and responds to departmental inquiries.
• Greets and attends patients; secures signatures for all required documentation; follows CHMG policies.
• Makes collection attempts while educating on resources available; reconciles work lists and reports.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information for new and existing patients, and creates and maintains medical records.
• Schedules, confirms or reschedules patient appointments; works Televox Reports daily; promotes Patient Connect enrollment.
• Supports a culture of "yes" in support of a patient centered medical home.
• Works independently and within a team on special, nonrecurring and ongoing projects; coordinates multiple aspects of projects, events, and other complex activities.
