Job summary

SCAN Health Plan is seeking a Bilingual Spanish Community Health Worker in Oceanside, CA. The role combines remote and fieldwork (75% in the field) to support frail seniors, navigate care, and implement personalized plans.

You will collaborate with the care team to address barriers, coordinate services, and empower members and families in managing health and social needs. The ideal candidate is bilingual, with a Community Health Worker background and strong communication skills, ready to travel

Qualifications

• Must be bilingual in English and Spanish.
• Experience in community engagement or case management is preferred.
• Knowledge of medical terminology and local senior resources helpful.

Responsibilities

• Provide person-centered care telephonically and in-person in Oceanside and surrounding areas.
• Conduct assessments to identify chronic conditions and psychosocial challenges.
• Coordinate care plans, navigation, and referrals to community resources.
• Support ECM activities for priority populations and document progress.
• Engage in interdisciplinary planning and maintain confidentiality.
