Job summary

SCAN Health Insurance is seeking a Field/Remote Care Coordinator to support frail seniors in aging in place. You will conduct assessments, coordinate care, and empower members and families via telephonic and in-person interactions in Oceanside, CA and surrounding areas.

The role emphasizes bilingual English/Spanish communication, travel to assigned communities, and collaboration within an interdisciplinary team to ensure person-centered, guideline-compliant care plans.

Qualifications

• High School Diploma or equivalent.
• Bilingual English/Spanish required.
• Willing to travel 50% in the assigned area.
• 1+ year of community engagement experience.
• Experience with managed care or case management preferred.
• 1+ year working with seniors, home visits, and remote work preferred.
• Basic knowledge of medical terminology and local resources.
• Knowledge of NCQA, CMS and DHCS standards preferred.

Responsibilities

• Conduct assessments to identify chronic conditions and psychosocial challenges.
• Provide care coordination and coaching to empower members and families.
• Perform Enhanced Care Management (ECM) activities for specified populations.
• Develop personalized care plans aligned with member goals.
• Document care plans and progress notes accurately and timely.
• Network with community organizations to support members.
