Job summary

SCAN Group in Los Angeles is seeking a bilingual Community Engagement Specialist to support frail seniors, working remote and in the field about 75% in South LA area. You will collaborate with care teams to implement plans and help members navigate care.

The role emphasizes field visits, coordination with community resources, and adherence to NCQA/CMS/HCS standards. Travel 50%+ with a valid license; bilingual English/Spanish required; experience with seniors and managed care preferred.

Qualifications

• High School Diploma or equivalent is required.
• Community Health Worker Certificate strongly preferred.
• Bilingual English/Spanish is required; tests may apply.
• Travel 50%+ with valid driver's license and reliable transport.
• 1+ year of community engagement experience.
• Experience with managed care or case management preferred.
• Experience with seniors, home visits, and remote work preferred.
• Basic technical skills for the functional area.
• Basic problem-solving skills and good communication.
• Confidentiality and discretion are essential.
• Familiarity with NCQA, CMS, and DHCS standards.

Responsibilities

• Conduct assessments of chronic conditions and psychosocial challenges.
• Provide field or telephonic care coordination and coaching.
• Support member well-being and help navigate primary and preventive care.
• Coordinate with community resources and referrals to services.
• Perform Enhanced Care Management activities for specific focus populations.
• Contribute to interdisciplinary planning and post-discharge follow-up.
• Engage with community organizations to expand support networks.
