SCAN Health Plan is seeking a Community Engagement Specialist to enhance frail seniors’ ability to age in place by delivering person-centered care and coaching. You will work telephonically and in-person to implement plans of care, advocate for members, and coordinate with healthcare and community resources.

Responsibilities include conducting assessments, supporting Medi-Cal/Medicare dual-eligibles, and performing ECM activities across focus populations such as individuals experiencing

Formación

• High School Diploma or equivalent experience.
• Community Health Worker Certificate strongly preferred.
• BILINGUAL required in English/Spanish.
• Travel 50%+ of the time in the assigned area; must have reliable transportation.
• 1+ years of Community Engagement Experience.
• Experience with managed care, healthcare environment, lived experience or case management strongly preferred.
• 1+ years working with seniors, conducting home visits, and working remotely strongly preferred.

Responsabilidades

• Conduct general assessments to identify chronic conditions and psychosocial challenges.
• Assist LTSS/Nursing Facility level of care determinations with RN support.
• Support member well-being through observation and motivational interviewing.
• Conduct face-to-face or phone visits to address care barriers and engagement in care.
• Provide care coordination and empower members and families through coaching and navigation.
• Perform ECM activities for various Focus Populations.
• Develop personalized care plans tailored to medical and social needs.
• Participate in interdisciplinary planning and case conferences.
