Job summary

SCAN is seeking a Remote/Field Community Engagement Specialist to support frail seniors, empowering them to age in place and navigate care systems. You will work telephonically and in-person with members to implement care plans, advocate for them, and provide member-centric interventions.

The role emphasizes collaboration with care teams, building relationships with community resources, and maintaining documentation per ECM requirements. Travel 50%+ is expected in the South Bay area.

Qualifications

• High School Diploma or equivalent required.
• BILINGUAL English/Spanish required; proficiency test may be administered.
• Travel 50%+ in assigned area; valid driver's license and reliable transportation.
• 1+ year of Community Engagement Experience.
• Experience with managed care or healthcare environment preferred.
• 1+ year working with seniors, home visits, and remote work preferred.
• Basic technical skills for the role.
• Strong communication and interpersonal skills.

Responsibilities

• Collaborate on general assessments to identify chronic conditions and psychosocial challenges.
• Perform Medi-Cal/Medicare LTSS referrals and assist Nursing Facility level of care.
• Support member well-being through observation and motivational interviewing.
• Coordinate care through coaching, education, and healthcare navigation.
• Lead Enhanced Care Management activities for specified populations.
• Develop personalized care plans and track progress with the team.
• Engage with community organizations to build local partnerships.
