SCAN Health Plan is seeking a bilingual Community Engagement Specialist to support frail seniors in their homes and community settings. The role emphasizes person-centered care, care coordination, and navigation of Medi-Cal/Medicare services.

You will perform assessments, collaborate with the care team, and advocate for members while coordinating with community resources; travel is required across the assigned area with remote work options.

Qualifikationen

• High School Diploma or equivalent experience.
• Community Health Worker Certificate strongly preferred.
• Bilingual English/Spanish required.
• Travel 50%+ in the assigned area; valid driver's license and reliable transportation.
• 1+ years of Community Engagement Experience.
• Experience with managed care, healthcare environment, lived experience or case management preferred.
• 1+ years working with seniors, conducting home visits, and remote work preferred.

Aufgaben

• Collaborate in assessments to identify chronic conditions and psychosocial challenges.
• Assist Medi-Cal/Medicare dual-eligible members for LTSS and support Nursing Facility Level of Care determination.
• Provide care coordination, coaching, and healthcare navigation for members and families.
• Conduct face-to-face or phone visits to address care barriers and support care plan implementation.
• Build relationships with community resources and document plans timely.
• Participate in interdisciplinary planning and case conferences to ensure person-centered care.
