Job summary

SCAN Health Plan is seeking a Field Care Coordinator to enhance frail seniors’ ability to age in place by providing person-centered care telephonically and in person. You will implement care plans, advocate for members, and support navigation within the healthcare system.

The role involves 75% remote/field work in West LA and surrounding areas, collaborating with a multidisciplinary team, and documenting progress in a timely manner.

Qualifications

• High School Diploma or equivalent.
• Community Health Worker Certificate preferred.
• Bilingual English/Spanish; testing may apply.
• 50%+ travel in assigned area; valid driver's license & insurance.
• 1+ year of Community Engagement Experience.
• Experience with managed care or healthcare settings preferred.
• Experience with seniors, home visits, and remote work preferred.
• Technical, problem solving, communication, and organizational skills.
• Ability to collaborate in a multidisciplinary team.
• Confidential handling of sensitive information and knowledge of NCQA/CMS/DHCS rules.

Responsibilities

• Collaborate in general assessments to identify chronic conditions and psychosocial challenges affecting independence.
• Assess Medi-Cal/Medicare dual-eligible members and assist RN in care determinations.
• Support member well-being through observation, communication, and motivational interviewing.
• Conduct visits (face-to-face or phone) to address care barriers and care plan implementation.
• Provide general care coordination and empower members and families to manage chronic conditions.
• Implement ECM activities for specified Populations of Focus.
• Develop personalized care plans aligned with medical and social needs.
• Participate in interdisciplinary planning and post-discharge follow-up.
• Maintain timely documentation of care plans and progress notes.
• Network with community organizations to support members.
• Identify barriers to work processes and raise to supervisor.
• Work under direct supervision; escalate complex issues as needed.
• Support SCAN’s Vision and Goals.
