Alignment Health is a fast-growing healthcare company focused on serving seniors and the chronically ill. This high-volume, phone-based role supports transitions-of-care after hospital and skilled nursing facility discharges, coordinating follow-up care and identifying needs.

The ideal candidate has at least one year of healthcare experience, completed an accredited Medical Assistant program, and is fluent in English and Spanish.

Qualifications

• Minimum 1 year healthcare experience and completion of an accredited Medical Assistant program.
• Bilingual English/Spanish required.
• Proficient with Microsoft Outlook, Excel, and Word.

Responsabilités

• Conduct high-volume outbound calls to members and complete time-sensitive outreach protocols.
• Engage members to schedule appointments, arrange referrals, authorizations, DME, medication refills, and document outcomes.
• Work as a team with clinical and care management staff and escalate clinical concerns appropriately.
