Suvida Healthcare is seeking a Care Coordinator to support high‑risk patient care. You will work with the Nurse Care Manager to manage daily follow-ups, DME and home health orders, and triage calls, ensuring timely, coordinated care for Medicare‑eligible Hispanic seniors.

Qualifications

• Minimum 3 years’ experience as a Medical Assistant or LPN / LVN.
• 5 years’ experience as a Medical Assistant preferred.
• Experience in chronic care management or related field preferred.
• Excellent interpersonal and communication skills.
• Strong organizational and time management abilities.
• Bilingual/Bicultural (English and Spanish) required.

Responsibilities

• Conduct daily patient follow-ups for high‑risk patients in chronic care programs.
• Process DME and home health orders efficiently.
• Support inbound triage calls and coordinate responses to acute needs.
• Retrieve hospital records for recently admitted patients.
• Perform clinical clerical tasks to support care programs.
• Collaborate with the Nurse Care Manager to ensure seamless care delivery.
• Identify barriers to care for high‑risk patients.
• Coordinate patient care progression across transitions of care.
