Job summary

Qualifications

Responsibilities

• Conduct daily patient follow-ups for high-risk patients within care programs.
• Process DME and home health orders accurately and promptly.
• Triage incoming calls and coordinate responses to acute patient needs.
• Retrieve hospital records for recently admitted patients and update records.
• Collaborate with the Nurse Care Manager to ensure seamless patient care delivery.
