Job summary

Suvida Healthcare LLC is seeking a Care Coordinator to join our primary care team serving Medicare-eligible seniors. The role focuses on daily patient follow-ups, coordinating DME and home health orders, triaging calls, and retrieving hospital records for recently admitted patients, under the Nurse Care Manager’s supervision.

You will work closely with a multidisciplinary team to ensure seamless patient care, identify barriers to care, and support Medicare HEDIS metrics while improving patient

Qualifications

• Minimum 3 years’ experience as a Medical Assistant or LPN / LVN.
• 5 years’ experience as a Medical Assistant preferred.
• Experience in chronic care management or related field preferred.
• Excellent interpersonal and communication skills.
• Strong organizational and time management abilities.
• Bilingual/Bicultural (English and Spanish) required.

Responsibilities

• Conduct daily patient follow-ups for high-risk patients in Chronic Care and Transition of Care Programs.
• Process DME and home health orders efficiently and accurately.
• Support inbound triage calls from patients and coordinate appropriate responses to acute patient needs.
• Retrieve hospital records for recently admitted patients from external facilities.
• Perform clinical clerical tasks to support Chronic Care Program and Transition of Care Programs.
• Assist in procedures within the Medical Assistant scope of practice for high-risk patients.
