Job summary

Alignment Health is seeking a bilingual English/Spanish Care Coordinator to join the remote Care Anywhere team. The role centers on transitions-of-care outreach after hospital or SNF discharges, conducting outbound calls, documenting activities, and coordinating follow-up care with providers.

This is a high-volume, phone-based position with Pacific Time scheduling (two options) and requires strong communication, documentation, and teamwork with clinical staff to ensure timely care.

Qualifications

• Minimum 1 year of health care experience with high-volume outbound calls.
• Preferred: transitions of care (TOC), post-discharge outreach, scheduling and authorizations.
• Able to work Pacific Time and communicate empathetically by phone.

Responsibilities

• Conducts high-volume outbound telephonic outreach to members following Hospital and SNF discharges.
• Follows established, time-sensitive outreach protocols and completes outreach and follow-up activities.
• Engages members to schedule timely follow-up appointments with Care Anywhere providers.
• Uses scripts and questionnaires to guide outreach and capture required information.
• Completes timely documentation of outreach, outcomes, and follow-up in required systems.
