Job summary

Suvida Healthcare is hiring a Care Coordinator to support high-risk patients with daily follow-ups, DME/home health orders, and triage duties. You will work closely with the Nurse Care Manager to ensure smooth patient transitions and care continuity.

The role requires bilingual English/Spanish proficiency, strong organizational skills, and experience in chronic care management. Excellent communication with physicians and care teams is essential.

Qualifications

• Minimum 3 years' experience as a Medical Assistant or LPN/LVN.
• 5 years' experience as Medical Assistant preferred.
• Experience in chronic care management or related field preferred.

Responsibilities

• Conduct daily patient follow-ups for high-risk patients in Chronic Care and Transition of Care programs.
• Process DME and home health orders accurately.
• Support inbound triage calls and coordinate responses to acute patient needs.
• Retrieve hospital records for recently admitted patients.
• Collaborate with Nurse Care Manager to ensure seamless patient care delivery.
• Coordinate care progression across acute, post-acute, and home settings.
