Job summary

Trilogy is hiring a Recovery Counselor to assess, plan, and support clients through home, community, and office visits in English and Spanish. You will coordinate recovery plans, document care, and provide crisis coverage.

Qualifications

• Bilingual in Spanish is required.
• Multiple educational/licensing options listed above may qualify.
• Clinical experience in mental health settings preferred.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits in English and Spanish.
• Coordinate establishment of an individual recovery plan with client, support system, and care providers.
• Complete daily progress notes and MAR documentation in a timely manner.
• Document care-related forms (IMCANS, LOCUS, tracking forms) in EMR.
• Provide crisis/on-call coverage as directed.
• Assist clients with entitlements applications (SSI, SSDI, Medicare/Medicaid, LINK) in English and Spanish.
• Advocate for clients and link to community services as needed.

Skills

Bilingual in Spanish

Education

Bachelor's degree in counseling and guidance or related human service field

Bachelor's degree with two years supervised clinical experience in mental health

Licensed Practical Nurse under the Nurse Practice Act

Certificate of Psychiatric Rehabilitation from a DHS-approved program

CRSS certification

Family Partnership Professional IAODAPCA certificate

Occupational Therapy Assistant license

High school diploma/GED with five years of supervised clinical experience
