Job summary

Trilogy, Inc. in Chicago, IL seeks a Recovery Counselor to assess clients across home, community, and office settings and plan individualized treatment courses.

You will deliver strengths-based, trauma-informed services, using harm-reduction approaches and motivational interviewing to support recovery from mental illness and co-occurring substance use disorders. The role emphasizes 75% community-based service delivery, thorough documentation, and crisis/on-call coverage.

Qualifications

• Bachelor's degree in counseling, social work, or related field.
• Valid Illinois driver’s license with daily access to a vehicle.
• Ability to work evenings and weekends as directed.
• Experience in mental health and/or substance abuse settings preferred.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate a recovery plan with client, support system, and care providers.
• Complete daily progress notes in EMR per policy and standards.
• Carry out documentation related to client care (IMCANS, LOCUS, MAR).
• Provide crisis/on-call coverage as directed.
• Advocate for clients and connect them to community services.
