Job summary

Trilogy, Inc. is seeking a Recovery Counselor in Chicago to assess clients’ personal, medical, emotional, social, and environmental needs to plan treatment across home, community and office settings.

You will advocate for clients, link them to services, and provide trauma-informed, strengths-based care for individuals with severe mental illness and co-occurring substance use disorders, using harm-reduction principles.

Qualifications

• Bachelor-level degree in counseling, social work, psychology, or related human service field or equivalent.
• Two years of supervised clinical experience in mental health if not holding a related degree.
• Licensed Practical Nurse licensed in Illinois or appropriate credential.
• CRSS and IAODAPCA credentials are preferred and may be required to obtain.
• Family Partnership Professional credential from IAODAPCA is preferred.
• Occupational Therapy related licenses/credentials with relevant experience are acceptable.
• High school diploma or GED with five years of supervised clinical experience also accepted.

Responsibilities

• Assess risk and determine level of care including hospital, detox, or ER when needed.
• Coordinate support with team to manage risk and arrange two-person visits when required.
• Assess clients across personal, medical, emotional, social, and environmental domains through various settings.
• Engage clients using assertive outreach to link them to treatment services.
• Participate in initial engagement with clients via street, shelters, jail, and community outreach in Chicago.
• Complete intake paperwork and facilitate service applications and consent forms.
• Conduct mental health assessments to determine needs and level of care; document encounters.
• Coordinate authorizations and liaise with managed care organizations.
• Develop individual recovery plans with clients and support networks.
• Identify opportunities to move clients to higher levels of care and refer to additional services.
• Help clients recognize signs of decompensation and develop coping skills.
• Assist with independent living skills such as hygiene, housekeeping, nutrition, and shopping.
• Assist clients with applying for entitlements (SSI, SSDI, Medicare, Medicaid, SNAP).
• Accompany clients to appointments and assist with housing, substance use, budgeting, social support, and medications.
• Advocate for clients and empower self-advocacy, including understanding the criminal justice system and housing options.
• Communicate with community partners and team members for coordinated care.
• Perform other related duties as assigned.

Education

Bachelor's degree or higher in counseling/related field
