Trilogy Behavioral Healthcare is seeking a Recovery Counselor to assess and support clients across personal, medical, emotional, and social domains. Location-based in Chicago, you will plan recovery through home, community, and office visits, providing motivational, strengths-based services with a harm-reduction approach.

You will manage a caseload under supervision from a Team Leader, coordinate recovery plans, and connect clients with community resources.

Competenze

• Must have one of several specified educational or credential requirements.
• Valid Illinois Driver’s License with daily access to a vehicle and $100,000/$300,000 auto liability insurance.
• Experience with mental health and/or substance abuse diagnoses preferred.
• Experience in community-based services preferred.

Mansioni

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and care providers.
• Identify opportunities to move clients to appropriate levels of care and provide referrals.
• Help clients recognize signs of de-compensation and develop coping skills.
• Assist clients in independent living skills such as hygiene, household tasks, nutrition, and shopping.
• Educate clients on entitlements (SSI/SSDI, Medicare, Medicaid, LINK).
• Accompany clients to appointments and support housing, budgeting, and medication needs.
• Advocate for clients and empower self-advocacy.
• Perform other related duties as assigned.

Formazione

Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field

Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting

Licensed Practical Nurse under the Nurse Practice Act

Certificate of Psychiatric Rehabilitation from a DHS-approved program

CRSS certification through IAODAPCA

Family Partnership Professional certificate from IAODAPCA

Occupational Therapy Assistant licensed

High school diploma or GED with five years of supervised clinical experience
