Job summary

Trilogy in Chicago is seeking a Recovery Counselor to assess clients’ medical, emotional, social, and environmental needs and plan treatment across home, community, and office settings.

Work with clients facing high barriers and criminal justice involvement, connecting them to community services, entitlements, and higher levels of care as needed. The role emphasizes trauma-informed, strengths-based care within a harm-reduction framework.

Qualifications

• Bachelor’s degree in counseling/related field or two years of supervised mental health experience
• Licensed Practical Nurse or equivalent in mental health setting
• IAODAPCA certifications or equivalents
• OT/related certifications optional with relevant experience

Responsibilities

• Independently assess risk and need for higher level of care
• Coordinate with supervisors to manage risk
• Assess client situations through home, community, and office visits
• Implement engagement strategies to bring clients into services
• Participate in outreach across Chicago
• Complete intake and client paperwork
• Conduct mental health assessments and document encounters
• Obtain authorizations for services and coordinate with managed care
• Develop individual recovery plans with client and support system
• Refer to additional services and facilitate housing, benefits, and transportation support
• Provide crisis on-call coverage as directed
• Assist clients with entitlements (SSI, SSDI, Medicare/Medicaid, SNAP)
• Support clients in independent living skills and coping strategies
• Accompany clients to appointments and advocate for housing and care options
• Collaborate with partners in a multidisciplinary care model
• Perform other duties as assigned

Education

Bachelor’s degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field

Bachelor’s degree in any other field with two years of supervised clinical experience in a mental health setting

Licensed Practical Nurse under the Nurse Practice Act [225 ILCS 65]

Certificate of Psychiatric Rehabilitation from a DHS‑approved program, a high school diploma/GED, and two years’ experience in a mental health setting

CRSS certification through IAODAPCA

Family Partnership Professional certificate from and in good standing with IAODAPCA

Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one‑year experience in a mental health setting

High school diploma or GED and five years of supervised clinical experience in a mental health setting
