Oak Street Health is seeking a Patient Growth Specialist (Outreach Executive I) to connect Medicare-eligible seniors with its care network. This community-facing role drives local patient growth by executing targeted outreach and events in the assigned market, collaborating with managers to expand access to care.

The position is 60% field-based and 40% clinic-based, with weekend opportunities for community events.

Qualifications

2+ years in Sales or Healthcare environments.
Fluent in English and Spanish.
Proven ability to cold call, manage digital lead channels, and influence key stakeholders.
Exceptional verbal and written communication skills with a professional presence.
Must have a valid driver’s license and reliable transportation.

Responsibilities

Generate high-quality leads by executing community events and localized outreach.
Identify and convert leads through cold calling, digital channels, and referrals.
Engage prospective patients via email, phone, and in-person networking to build a sales pipeline.
Stay current on service updates and marketing initiatives through team briefings.
Partner with CRMs and leadership to align on lead generation strategies.
Document all customer interactions in the CRM and internal systems.
Serve as liaison between clinical teams and new patients to ensure a seamless transition.
Address and resolve prospect concerns with empathy to create a positive first impression.
