Job summary

Elevance Health is seeking a Field-based LTSS Service Coordinator to manage a defined caseload of individuals in specialized programs across multiple Florida counties. The role focuses on Person Centered Planning, assessments, and coordinated care for LTSS, Behavioral Health, and Medical Health needs.

Ideal candidates hold a BA/BS with at least 2 years of social work experience and are fluent in English and Spanish.

Qualifications

• BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Perform face to face program assessments using predefined questions and motivational interviewing techniques.
• Coordinate and manage an individual’s waiver (LTSS/IDD), BH or PH needs and document goals in the Person Centered Plan.
• Interface with Medical Directors and participate in interdisciplinary care rounds to support an integrated care plan.
• Engage the member’s circle of support (caregivers, family, natural supports, service providers, physicians).
• Identify members who may benefit from alternative levels of service or other waiver programs.
• May serve as mentor or preceptor for new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with documentation supporting the care plan.
• Report critical incidents to appropriate state and county agencies and participate in appeals or audits.
