Job summary

Elevance Health seeks a field-based LTSS Service Coordinator to manage a defined caseload and coordinate care across PH/BH/LTSS needs. The role involves in-person assessments, collaborating with care teams, and documenting member goals with their support circle.

Travel to client sites is required within Miami-Dade or Monroe counties. The position requires a BA/BS and at least 2 years in a social work setting; bilingual English/Spanish strongly preferred.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Experience with LTSS, case management or hospital discharge planning preferred.
• Bilingual in English/Spanish strongly preferred.

Responsibilities

• Conduct face-to-face program assessments for LTSS/IDD, BH or PH needs.
• Coordinate cases with clinical teams as the single point of contact.
• Document member goals in collaboration with their care team.
• Identify members who may benefit from alternative service levels or other waivers.
• Assist in training new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies as required.
• Support member appeals, grievances and state audits.
