Job summary

Elevance Health is seeking a LTSS Service Coordinator (Bilingual in Spanish/English) to support a defined caseload in the field, primarily residing in Hardee, Highlands, Hillsborough, Manatee, or Polk County, FL. This role travels to client sites, with some office attendance, under a hybrid/work policy.

You will conduct assessments, develop Person Centered Plans, and coordinate care with medical directors and an interdisciplinary team to deliver integrated PH/BH/LTSS support.

Qualifications

• BA/BS degree required or equivalent experience.
• Bilingual in English/Spanish strongly preferred.
• Experience in LTSS, case management, social work or hospital discharge planning preferred.

Responsibilities

• Perform face-to-face program assessments using predefined questions and motivational interviewing to manage LTSS/IDD and BH/PH needs.
• Coordinate members'' cases with clinical healthcare management and interdisciplinary teams as the single point of contact.
• Manage non-clinical needs of members with chronic illnesses or disabilities to optimize LTSS utilization.
• Document short and long-term service goals with the member and care team (caregivers, family, providers).
• Identify members who may benefit from alternative services or other waiver programs.
• Submit utilization/authorization requests and report critical incidents to state/county agencies.
• Assist with appeals, grievances, and state audits as needed.
