Job summary

Elevance Health is seeking a LTSS Service Coordinator (Bilingual Spanish/English) to manage a defined caseload with a focus on comprehensive person-centered planning and inter-disciplinary care coordination in Hillsborough County, FL.

You will assess needs, coordinate LTSS/IDD and BH/PH services, and document goals within PCSP while collaborating with the member's support circle. Travel to client sites is required as part of field-based duties.

Qualifications

• Requires BA/BS degree and at least 2 years in social work or equivalent.
• Bilingual in English/Spanish strongly preferred.
• LTSS, case management, social work or hospital discharge planning exp. preferred.

Responsibilities

• Conduct face-to-face program assessments using standardized questions and tools.
• Coordinate waiver care (LTSS/IDD) and BH/PH needs with the clinical team.
• Document member goals in PCSP and backup plans with the care team.
• Submit utilization/authorization requests and support care plan alignment.
• Assist in training/mentoring new staff and participate in process improvements.
