Job summary

Elevance Health seeks a LTSS Service Coordinator (Bilingual Spanish/English) to support a defined caseload, conduct assessments, and develop a Person Centered Support Plan with the member and care team in collaboration with providers.

The role involves field travel to client sites, coordination across PH/BH/LTSS needs, and working within state requirements. Requires BA/BS and 2+ years’ experience; bilingual ability strongly preferred.

Qualifications

• BA/BS degree and a minimum of 2 years of experience working with a social work agency.

Responsibilities

• Conduct face-to-face program assessments and apply motivational interviewing techniques.
• Coordinate member cases with clinical healthcare management and interdisciplinary teams.
• Manage non-clinical needs of members to ensure efficient utilization of LTSS.
• Document short and long-term service goals with the member and care team.
• Identify members who may benefit from alternate levels of service or other waiver programs.
• Support training of new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies as required.
• Assist with member grievances, appeals, and state audits.
