Job summary

Elevance Health seeks a LTSS Service Coordinator - Case Manager (Region D: Highlands/Hardee) in Florida. This field-based role conducts assessments, develops person-centered plans, and coordinates LTSS/BH/PH needs with clinical teams and the member's circle of support.

The role requires relocation/ residence within Hardee, Highlands, Hillsborough, Manatee, or Polk counties and a strong emphasis on field engagement, with occasional office meetings. Bilingual in Spanish/English preferred.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.

Responsibilities

• Conduct face-to-face program assessments and coordinate waiver services (LTSS/IDD) and BH/PH needs.
• Serve as single point of contact, coordinating with clinical teams to provide care coordination support.
• Document member goals in the care plan with the care team and circle of support.
• Identify members who may benefit from alternative service levels or other waivers.
• Support training and process improvement initiatives, and assist in appeals and audits.
