Job summary

Elevance Health is seeking a LTSS Service Coordinator to coordinate care for individuals in specialized programs. The role focuses on Person Centered Planning, assessment, and development of comprehensive support plans, collaborating with Medical Directors and an interdisciplinary team.

The position requires a BA/BS in a health-related field and at least 2 years of relevant experience. The ideal candidate is bilingual in English/Spanish and capable of engaging family and care teams to support

Qualifications

BA/BS degree in a health-related field is required.
2+ years of experience with a social work agency or equivalent background is preferred.

Responsibilities

Conduct face-to-face program assessments using predefined questions and motivational interviewing.
Identify members with potential clinical health care needs and coordinate with the clinical team.
Manage non-clinical needs of members with chronic illnesses to ensure efficient utilization of LTSS.
Document short and long-term service goals with the member and care team.
Identify members who may benefit from alternative service levels or other waiver programs.
Mentor or train new staff and contribute to process improvement initiatives.
Submit utilization/authorization requests aligned with the care plan.
Report critical incidents to applicable agencies and participate in appeals and state audits.
