Qualifications

• BA/BS degree and a minimum of 2 years of experience with a social work agency.
• Education and experience variations may be considered to meet equivalent background.

Responsibilities

• Conduct face-to-face program assessments and coordinate LTSS/IDD and BH or PH needs.
• Identify members with potential clinical health care needs and coordinate with the interdisciplinary team.
• Manage non-clinical needs of members with chronic illnesses or disabilities to ensure efficient utilization of LTSS.
• Document member goals with the care team, including caregivers and providers.
• Identify members who may benefit from alternative service levels or other waiver programs.
• Mentor or preceptor roles for new staff and contribute to process improvements.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state/county agencies and other external parties.
• Assist with appeals, grievances, and state audits as needed.
