Job summary

Mission Neighborhood Health Center (MNHC) in San Francisco seeks a Service Coordinator – Housing Focused (Bilingual). The full-time, non-exempt position assists individuals experiencing homelessness with housing applications, plans, and referrals, coordinating with housing providers and MNHC programs.

You will document care in the HSH ONE System, maintain HIPAA-compliant records, and deliver trauma-informed, culturally responsive services in English and Spanish.

Qualifications

• High school diploma or equivalent required.
• Bilingual in English and Spanish required.
• Experience in case management, housing navigation, intake and assessment helpful.

Responsibilities

• Provide one-on-one housing navigation services and develop individualized housing plans.
• Assist clients with completing housing applications and gathering required documentation.
• Maintain communication with clients and coordinate with landlords, housing providers, and partner agencies.
• Maintain accurate, confidential records in the HSH ONE System and other databases per HIPAA standards.
• Deliver trauma-informed, culturally responsive services in both English and Spanish.
