Job summary

Mission Neighborhood Health Center is seeking a Service Coordinator - Housing Focused (Bilingual) to assist individuals experiencing or at risk of homelessness with housing navigation, applications, and individualized housing plans. You will coordinate with housing providers, maintain accurate records, and offer culturally responsive services in a trauma-informed manner.

The role requires English and Spanish fluency and reports to the Program Manager.

Qualifications

• Bilingual in English and Spanish required.
• Two years of experience in case management, housing navigation, intake and assessment, or related direct services.
• Proficiency with MS Office; ability to learn electronic case management systems like ONE System or Epic.

Responsibilities

• Provide one-on-one housing navigation and develop individualized housing plans.
• Assist clients with housing applications and required documents.
• Maintain confidential client records per HIPAA and regulatory standards.
• Coordinate with landlords, housing providers, and partner agencies for placements.
• Communicate updates and barriers to supervisors and care teams.
