Job summary

MNC is seeking a Housing Case Manager I to serve households at risk of homelessness at the MNC Evans Campus in San Francisco. You will coordinate housing-focused case management, connect clients to legal services, mediation, benefits and behavioral health resources, and maintain confidential records.

The role requires 40 hours weekly, with potential evenings and weekends, and a preference for bilingual English/Spanish speakers.

Qualifications

• Bachelor’s Degree in Education, Human Services, Social Work, Psychology or related field or equivalent work experience.
• 6 months experience with clients from low-income or impacted backgrounds; homelessness, DV, covid recovery; SF ERAP experience a plus.

Responsibilities

• Provide Housing Focused Case Management and connect households with community resources.
• Develop and implement Housing Stability Plans with households.
• Refer non-eligible households to appropriate financial/housing resources.
• Input data into required systems and prepare monthly/quarterly/annual reports.
