Mission Neighborhood Health Center in San Francisco is seeking a Bilingual Care Manager - Resource Center to work with a multidisciplinary team, providing ongoing case management, crisis counseling, and links to housing, immigration, and benefits services. The role supports assessment, linkage to mental health and substance use programs, and care coordination to promote health equity.

Qualifications

• Associate’s degree in social work, psychology, counseling or related field or 3 years of paid work with unhoused adults with multiple diagnoses.
• Bilingual in Spanish and English.
• Excellent interpersonal communication and writing skills.
• Ability to maintain confidentiality in a multi-cultural team setting.

Responsibilities

• Perform psychosocial assessments of Resource Center clients and determine eligibility for case management, medical, and psychotherapy.
• Develop care plans with clients and coordinate with the integrated services team and referrals.
• Provide insight-oriented, harm-reduction based counseling to individuals, families, and groups.
• Provide crisis intervention and assess suicide risk and safety needs.
• Utilize evidence-based tools (PHQ-9, SBIRT) and connect clients to housing, immigration and public benefits services.
• Assist clients with documentation and forms for housing, benefits and eligibility; accompany to appointments as needed.
• Maintain documentation for case management activities and conduct follow-up contacts.
