Job summary

Mission Neighborhood Health Center in San Francisco is seeking a qualified Bilingual Care Manager to provide ongoing case management and crisis counseling at the Resource Center. You will work with a multidisciplinary team to connect participants with housing, immigration and public benefits, and coordinate with medical and behavioral health services.

The role requires a relevant degree or equivalent experience, bilingual (Spanish/English), and strong interpersonal skills.

Qualifications

• Associate’s degree in social work, psychology, counseling, or related field.
• Three years of paid work experience with unhoused adults with multiple diagnoses.
• Bilingual in Spanish and English.
• Excellent interpersonal communication and writing skills.
• Ability to work in a multi-cultural team with confidentiality.

Responsibilities

• Conduct comprehensive psychosocial assessments and determine care management eligibility.
• Develop and monitor individualized care plans with clear steps and goals.
• Provide crisis counseling and apply harm-reduction approaches.
• Perform suicide risk assessments and crisis intervention as needed.
• Coordinate housing, immigration and public benefits connections for clients.
• Assist with access to medical and behavioral health services and referrals.
