Job summary

Harvest Healthcare is seeking a Part-time Patient Care Navigator (Housing Navigation Specialist) to assist ECM members facing homelessness or housing instability. You will conduct field-based and telephonic outreach, coordinate housing resources, and support medical care access in collaboration with care teams.

The role emphasizes bilingual English/Spanish communication, strong boundary setting, and liaison with housing partners to secure services and stable housing for clients in Monterey

Qualifications

• Provide housing navigation and case management support to ECM members experiencing homelessness or housing instability.
• Telephonic and field-based outreach to engage clients in our care management program; prior experience with homelessness, domestic violence, or substance abuse communities is a plus.
• Ability to establish boundaries while providing supportive services and to maintain relationships with partners and patients.

Responsibilities

• Navigate housing process from identifying needs to applications and referrals.
• Coordinate with Care Team to connect members with appropriate resources and services.
• Help schedule and attend primary care appointments to review and update care plans.
• Identify mental health or psychosocial needs and arrange appropriate referrals.
• Maintain clear, concise client case records in the database.
• Provide health education to promote self-management in patients.
• Bilingual communication in English and Spanish.
