Addus HomeCare in Espanola NM is seeking a driven Service Coordinator to schedule and supervise in-home care workers and clients across a geographic area. The role focuses on keeping care plans on track, supporting field staff, and traveling for supervisory visits within a 30-mile radius.

The position requires strong organizational and communication skills, a high school diploma, and willingness to travel. In-office hours are Monday–Friday, 8am–5pm, with benefits and growth opportunities.

Qualifications

• Interpersonal, organizational and communication skills.
• Experience with scheduling in-home care or related field preferred.
• Proficiency in MS Word and Excel; ability to use scheduling software.

Responsibilities

• Creates work schedules by entering schedules into the system and updating them as needed.
• Provides alternate coverage to ensure the client’s care plan is followed.
• Contacts care providers and clients to give service updates.
• Conducts monthly client wellness calls and home visits as required.
• Supervises direct service employees and enforces company policies.
• Maintains a professional, clean, organized branch environment.
• Keeps information confidential and complies with regulations.
