Job summary

Ambercare in Espanola, NM is seeking a Service Coordinator to schedule caregivers, supervise in-home staff, and keep care plans on track. You will coordinate client changes, provide service updates, and ensure timely visits within a 30-mile radius.

The role is full-time, Monday–Friday, 8am–5pm, with travel required for supervisory visits. Supervisory or home care experience is preferred, and you must have reliable transportation.

Qualifications

• High school diploma or equivalent required.
• Minimum 1 year of industry experience.
• Strong interpersonal and communication skills.

Responsibilities

• Create and manage staff schedules in the scheduling system.
• Provide supervisor coverage to ensure care plans are followed.
• Coordinate with clients and care providers for updates.
• Conduct monthly client wellness calls and home visits as required.
• Address escalated client complaints and theft claims.
• Supervise direct service employees and enforce policies.
• Maintain a professional branch environment.
• Ensure compliance with MCO, Medicare, Medicaid and HIPAA.
• Travel to Espanola, Thruchas, and Los Alamos as needed.
