Job summary

The Project Access Nashville Specialty Care program, a project of the Medical Foundation of Nashville and Nashville Academy of Medicine, seeks a bilingual Care Navigator to provide patient-centered case management for Nashville’s uninsured at 200% of poverty or below.

This role connects patients with internal and community resources, requires English/Spanish proficiency, and collaborates with clinic staff to ensure timely access to specialty care.

Qualifications

• Minimum of a two-year post-secondary education; four-year degree preferred.
• Fluency in both Spanish and English (written and verbal).
• Customer service experience preferred (1 year).
• Proficiency with Microsoft Office (Word, Excel, etc.) and basic computer skills.

Responsibilities

• Serve as front-line liaison and advocate for uninsured patients.
• Connect patients to internal and community services to improve health and well-being.
• Coordinate referrals from clinics with attention to detail and follow-through.
• Engage in outreach for follow-up and patient communication.
• Maintain confidential information and protect patient privacy.
