Job summary
Legacy Community Health, a leading community health center in Houston, seeks a Bilingual Care Team Assistant to connect patients with resources and support their care journey. You will be the first point of contact, ensuring compassionate, accurate information and efficient triage with nursing staff.
Role requires Spanish fluency, high-school education or equivalent, and medical assistant certification or relevant clinical experience.
Qualifications
- Spanish fluency required.
- Certification as Medical Assistant or 2 years clinical experience preferred.
- BLS/CPR certification required.
- Experience in ambulatory clinical settings recommended.
- Excellent communication and customer service skills essential.
Responsibilities
- Handle patient communication via phone and in person, routing clinical inquiries to providers.
- Assist with pre-visit planning, reviewing schedules and arranging interpretation services.
- Support care coordination and transitions of care.
- Assist with medication refills and prior authorizations.
- Prepare forms for review and signature by providers; promptly address urgent requests.
- Process referrals and ensure documentation is included in patient records.
- Adhere to safety training and ensure equipment is properly used.
