Qualifications

  • High school diploma or equivalent.
  • Bilingual English/Spanish proficiency required.
  • 2+ years in a related role within a healthcare organization preferred.
  • Willing to complete a 20+ hour online training course and pass final exam for Federal certification prior to Certified Application Counselor duties.
  • Ability to work flexible hours, including evenings and weekends as needed.

Responsibilities

  • Performs patient-facing duties with focus on care quality and service.
  • Conducts patient service calls to ensure quality experience.
  • Schedules appointments and provides information for tests.
  • Refers patients to internal or external services as needed to address care needs.
  • Identifies resources to overcome barriers to care (transportation, housing, insurance).
  • Explains Marketplace enrollment processes and program options to clients.
  • Explains health packages to uninsured patients and facilitates enrollment where eligible.
  • Ensures payments and scheduling for initial wellness exams per contracts.
  • Collects data and reviews feedback for care improvements.
  • Collaborates with Training & Education to improve patient experience.
