Job summary

CAPS & Maple City seeks a Health Care Navigator to help patients access Medicaid, the Marketplace, and affordability resources, while maintaining Indiana Navigator certification. The role emphasizes documentation in EMR, coordination with the care team, and community outreach.

Qualifications

• Associate degree strongly preferred, especially in social work or related field.
• Certification as an Indiana Navigator is required.
• Certified Application Counselor (CAC) required, training provided.

Responsibilities

• Assist patients in accessing insurance information and Medicaid.
• Maintain certification as an Indiana Navigator.
• Assist patients in accessing in-house affordability resources.
• Assist patients access health care financial assistance programs through Goshen Health.
• Participate in community events to educate people about CAPS & Maple City's navigator services.
• Document patient contact using electronic medical record and other EMR documentation methods.
• Coordinate and follow up with care team to support patient access and continuity of care.
