Job summary

Mercy Community Healthcare in Franklin, TN is hiring a full-time Patient Navigator to help patients and families access services, coordinate care, and advocate for needs. You will facilitate referrals between primary care and subspecialists, link patients with social service agencies to address SDOH needs, and ensure timely documentation.

Candidates should have a high school diploma (Bachelor's preferred), strong communication, and comfort working with diverse populations; bilingual

Qualifications

• Must have a High School Diploma or equivalent with at least 1 year of medical office experience preferred.
• Bachelor's preferred in a medical or social service field.
• Bilingual (English/Spanish) highly preferred; strong communication and multitasking skills.

Responsibilities

• Serve as a patient ambassador to help patients and their families receive services.
• Facilitate referrals between primary care and subspecialists.
• Link patients with social service agencies to address SDOH needs.
• Maintain and update all patient interactions through proper and timely documentation.
• Advocate for needed services on behalf of patients, such as assistance with medication costs or transportation barriers.
• Help patients apply for financial assistance and reduce barriers to care.
• Treat patients with respect and dignity, portraying cultural competency in your awareness of the diverse needs of the patients served.
