Job summary

Pair Team in California seeks a Field Care Coordinator to deliver in-home and community-based care to high-need members. You will visit patients 3–5 times weekly, support care plans, and coordinate with the care team.

The role requires in-person field work with regular travel in Orange County, 9:00–17:30 hours, includes gas reimbursements. Fluency in English and Spanish is mandatory, and you’ll ensure timely documentation and patient follow-up.

Qualifications

• Bilingual – English/Spanish required.
• 1+ years of field (in person) case management or care coordination.
• Physical location in or near Orange County with familiarity of the local community and resources.
• Reliable transportation for regular field visits to patient homes, clinics, and partner sites.
• Valid driver’s license and current auto liability insurance.
• Strong cultural fluency and ability to build trust with diverse populations.
• High degree of empathy and ability to remain patient.
• Ability to work collaboratively in a multidisciplinary care team.
• Excellent organizational skills and attention to detail.
• Comfort learning new systems; experience with Google Suite, Slack, CRM, Excel, Word, email, and video conferencing.
• Reliable, HIPAA-compliant workspace with stable internet for documentation.

Responsibilities

• Serve as the primary contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, in the community, or at partner organizations for onboarding and care package delivery.
• Conduct in-person visits to assess needs and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across care teams.
• Collaborate with Nurses and Behavioral Health Care Managers to ensure care continuity.
• Identify and address barriers to care such as transportation and health literacy.
• Develop and maintain individualized care plans with follow-up on interventions.
• Maintain accurate documentation of member interactions and care activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in collaborative care meetings to review patient panels and plans.
