Job summary

Pair Team is seeking a Bilingual Field Care Coordinator to deliver care directly to members in homes and community settings across Orange County, CA. This full-time, field-based role involves regular travel and hands-on patient support to onboard new members and manage ongoing care.

The ideal candidate will have 1+ year of field case management, Spanish fluency, reliable transportation, and strong collaboration skills with nurses and care teams.

Qualifications

• 1+ years of field (in person) case management or care coordination.
• Physically located in or near Orange County with familiarity of local resources.
• Reliable transportation for regular field visits to homes, clinics, and partner sites.
• Valid driver’s license and current auto liability insurance.
• Spanish fluency required.
• Strong empathy and ability to build trust with diverse populations.
• Experience with Google Suite, Slack, CRM databases, and basic Excel/Word/email/video tools.
• HIPAA-compliant workspace and secure documentation practices.

Responsibilities

• Serve as the primary contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes or community sites to onboard new patients and deliver care packages.
• Conduct in-person visits to assess needs and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across care teams.
• Collaborate with Nurses and Behavioral Health Care Managers to ensure continuity of care.
• Identify barriers to care and assist with transportation, access, and health literacy.
• Develop and maintain individualized care plans and track interventions and goals.
• Document all member interactions and care coordination activities in the CRM.
